Health Plan/Home Health Partnership

The Health Plan/Home Health Partnership, co-chaired by MAHP and the Home Care Alliance of Massachusetts, was established in 1996 to provide a forum for health plans and home health agencies to exchange information and discuss any concerns or issues. The group meets several times a year and works to find common ground on common problems. The success of the Partnership is due in large part to the consensus approach and dedication of its members who are listed below. The overriding goal of the Partnership is to provide the best care for the patient.

 

The Partnership hosts “Connecting the Dots” conferences to bring the information exchange of the Partnership members to a wider audience of health plan and home health staff.

 

The Universal Authorization Form is a standard form created by the Partnership and used by health plans in Massachusetts to authorize home health services. The universal form has helped to simplify coordination between case managers and home health nurses by standardizing the information health plans require and eliminating the need for home health agencies to complete multiple forms.
